Toward Personalized Deep Brain Stimulation for Neuropsychiatric Disorders.

Personalized deep-brain stimulation (DBS) based on subthalamic nucleus (STN) electrophysiology has recently been proposed to improve clinical outcomes of DBS for Parkinson’s disease (PD) patients and other neuropsychiatric disorders. We have investigated STN neurophysiological biomarkers related to emotional and cognitive functions in PD and obsessive-compulsive disorder patients. We have used (i) rest and functional electrophysiology and (ii) acute and long-term recordings. While many current models for adaptive DBS are based on one or two electrophysiological features of STN activity, such as beta or gamma activity, we hypothesize that an adaptive DBS model for both neurological and psychiatric disorders that includes many STN activity parameters will yield better clinical results.
